Background

Autoantibodies to neutrophil cytoplasmic antigens (ANCA) play a central role in the evaluation of small-vessel vasculitides, including granulomatosis with polyangiitis (GPA), microscopic polyangiitis (MPA), and eosinophilic granulomatosis with polyangiitis (EGPA). The principal target antigens in ANCA-associated vasculitis (AAV) are proteinase 3 (PR3) and myeloperoxidase (MPO). Another form of vasculitis of the small blood vessels is anti-GBM disease (Goodpasture syndrome), which affects the glomerular and, in some cases, also the pulmonary capillaries. Characteristic autoantibodies are directed against the glomerular basement membrane (GBM). Up to 35 % of anti-GBM positive samples are also ANCA positive.5 Due to the clinical overlap of AAV and anti-GBM disease, it is recommended to investigate both anti-GBM antibodies and ANCA in patients with corresponding kidney damage.2

Diagnostics

Euroimmun provides a comprehensive diagnostic portfolio combining cell-based immunofluorescence assays (IFA) with high-quality antigen-specific assays (ELISAs), in line with international AAV consensus recommendations.5 IFA is performed on granulocytes fixed with ethanol (EOH) and formaldehyde (HCHO), enabling reliable differentiation of cytoplasmic (cANCA), perinuclear (pANCA) and atypical (xANCA) fluorescence patterns.

To further enhance diagnostic confidence, Euroimmun offers EUROPLUSâ„¢ BIOCHIP technology, which combines granulocyte substrates with defined single antigens such as PR3, MPO, and GBM on one reaction field, simplifying interpretation and supporting rapid correlation between patterns and antigen specificity.
