Testosterone is the male’s primary sex hormone that is produced in the leydig cells of the testes. However, it’s also produced in women in the ovaries and synthesis in small amounts also takes place in the adrenal cortex. In blood, most of it is bound to a protein called sex hormone binding protein (SHBH). The remaining is bound to albumin and a little percentage of it (2 to 3% ) is in free state. Remember, free and albumin-bound testosterone are considered biologically active. At older age, the concentration of free testosterone is significantly decreased due to increased synthesis of (SHBH), as more testosterone is bound to it.

Diagnostic Guidelines

Estimation of testosterone levels is considered in males that show hypo-function of the gonads (hypogonadism). The cause can be primary (originating in the testes themselves) or secondary (related to hypothalamus or pituitary). Whatever the cause, the concentration of testosterone in these men is significantly decreased. In men with the total testosterone values near the lower reference range or in those with suspected SBHB changes, free testosterone levels are considered.

In women, testosterone levels estimated in cases of virilization. The cause can be variable including:

  • Tumor in the ovary or adrenal cortex
  • Ovarian syndrome
  • Congenital adrenogenital syndrome

One thing to keep in mind is that testosterone levels are subject to variations based upon the time if the day. Generally, they come highest in the mornings.
