Dihydrotestosterone is made from testosterone via an enzymatic conversion involving the microsomal enzyme, 5-alpha reductase. It is the strongest androgen in humans and ranks fourth on blood concentrations of androgens after DHEA, androstenedione and testosterone.

The conversion of testosterone to dihydrotestosterone (DHT) occurs at different sites such as skin, prostate, liver and hair follicles. Once synthesized, it is released in the blood where, like testosterone, it binds to sex hormone binding protein. Its affinity for the protein however is 2 to five folds greater than that of testosterone. DHT is active male sex hormone that is responsible for male hair growth pattern and it also upkeeps the libido and male sexual functions.

Diagnostic Guidelines

The levels of DHT are predictors of various conditions.

  • Males with metabolic syndrome or type 2 diabetes have low levels of DHT and testosterone.
  • Men with 5-alpha reductase deficiency have low DHT and high testosterone levels. They have less pronounced external genitalia.
  • Increased DHT values are associated with hair loss (alopecia in men)
  • Women with hirsutism (male like hair distribution pattern) have high levels of DHT.
