Goodpasture syndrome also called pulmonary renal syndrome, is a disease of autoimmune etiology whereby special antibodies are formed against the alveolar (lung) and glomerular (kidney) basement membranes. Talking strictly about kidneys, this disease causes rapidly progressive glomerulonephritis characterized by rapidly declining kidney functions. Goodpasture syndrome affects men six times more than women, especially those in the younger segment of the population curve and is clinically detected by a combination of renal and pulmonary symptoms.
The primary target of the anti-GBM antibodies is the alpha-3 chain of type 4 collagen in the glomerular basement membranes. These antibodies are mostly of IgG class and can be detected qualitatively and quantitatively through IFA and quantitatively through ELISA. High titers hold a high predictive value and are positive in 60 percent of the patients with the disease limited to only kidneys and 90% of the patients with lung involvement too.
Of special importance is the association of the disease with ANCA (>35%). Hence individuals positive for serum ANCA should also be tested for anti-GBM antibodies to diagnose Goodpasture syndrome in the bud, if any.
