Disease information

Autoimmune encephalitis, as the name indicates, is an immune disease directed against neuronal cell surface antigens. Most of the antibodies are directed against glutamate receptors (both type NMDA and AMPA), GABAB receptors, voltage gated potassium channels and associated proteins (LGI1, CASPR2, TAG1/Contactin-2). Since these antigens are involved in synaptic transmission in one way or the other, autoimmunity against them breeds seizures and neuropsychiatric manifestations. The resulting conditions include:

  • Autoimmune limbic encepthalitis
  • Neuromyotonia
  • Morvan’s syndrome

All these syndromes are potentially lethal and they may be associated with a variety of para and non-paraneoplastic syndromes. 10 to 70 percent cases have an underlying tumor. The determination is based on the type of antibody present. Anyhow, since appropriate therapies are available including both immunotherapies and tumor resection procedures, and they result in significant regression in the symptoms, an early diagnosis is the key to a favorable prognosis.

Diagnostic Guidelines

The diagnosis of autoimmune encephalitis is made on a combination of clinical picture and supportive findings like brain MRT, EEG, CSF analysis and serological test for determination of antibodies. Monospecific assays for determination of serum antibody levels are by far the best methods for disease diagnosis. However, they are combined with a variety of conventional immunohistochemical detection procedures.

The overall diagnostic aim is also to rule out differentials such as infectious encephalitis (especially HSV) and other similar conditions of the central and peripheral nervous systems. One should keep in mind the possibility of overlap of syndromes. Hence, if a patient turns out positive for antibodies on serology, we also recommend comprehensive tumor investigations.
