Disease information

Connective tissue diseases (CTDs) are inflammatory rheumatic diseases based upon development of antibodies against body’s specific target antigens. The spectrum of these diseases includes:

  • Systemic lupus erythematous (SLE)
  • Progressive systemic sclerosis
  • Sjögren’s syndrome
  • Sharp syndrome

The hallmark of connective tissue diseases is the presence of antibodies directed against nuclear antigens. These antibodies are called ANAs (anti-nuclear antibodies) and are serological markers for CTDs. The target antigens can be:

  • Nucleic acids
  • Cell nuclear proteins
  • Ribonucleoproteins

These antibodies are implicated in 20 to 100% of the cases. Therefore, differential ANA diagnostics is an indispensable parameter for diagnosis of connective tissue diseases and their differentiation from other autoimmune diseases.

Diagnostic Guidelines

The gold standard for the detection of ANAs is indirect immunofluorescence (IFA) with human epithelial cells (HEp-2). This technique is both highly sensitive and specific. Moreover, microscopic analysis displays particular cellular patterns of the immunofluorescent conjugate bound to auto-antibodies, each dependent upon the location of specific target antigens.

Although, standardized protocols require additional testing of the antibodies using specific tests like ELISA and line blot, once they are detected on IFA, the fact that all nuclear antigens can not be obtained in purified samples as yet makes IFA the only reliable test for the diagnosis of connective tissue diseases.
