Disease information

Respiratory infections are mainly caused by viruses (influenza viruses, adenoviruses and RSV) or bacteria (legionella, bordetella pertussis, mycoplasma pneumoniae etc.). They can involve both the upper respiratory tract including the nasal mucosa and paranasal sinuses, and the lower respiratory tract all the way up to bronchi and bronchioles. Apart from a cough and other respiratory manifestations such as pneumonia, these also cause pharyngitis, fever and headache (due to sinusitis).

Diagnostic Guidelines

Since most respiratory pathogens cause a similar set of symptoms, diagnosis on the basis of clinical presentation alone becomes fairly difficult. Hence, lab diagnosis plays a pivotal role in ruling out differentials and making an accurate diagnosis. Direct pathogen detection (e.g. PCR) remains the method of choice for detection of acute respiratory infections.

Antibodies against the pathogens only appear days to weeks after the initial infection. Thus, infections are diagnosed retrospectively by serology and are indicated by a raised IgG titer. Serology also helps to monitor the disease activity and evaluate treatment response.
